The covered entity (CE), UnitedHealth Group, reported that an employee of its business associate (BA) mistakenly mailed the protected health information (PHI) of 896 individuals to the wrong recipients. The PHI involved included names, dates of birth, medical information, and financial and health insurance information. The CE notified HHS and the affected individuals and provided complimentary credit monitoring services. In its mitigation efforts, the BA implemented additional technical safeguards to better protect PHI and retrained its staff on the requirement to safeguard PHI.
