United Cerebral Palsy Heartland

The covered entity (CE), United Cerebral Palsy Heartland, reported that an employee was the subject of an email phishing incident that affected the protected health information (PHI) of 576 individuals. The PHI involved included names, dates of birth, diagnoses, medications, health insurance information Social Security and drivers’ license numbers, financial information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards and retrained all employees to better protect its PHI.
