UNC Lenoir Health Care

The covered entity (CE), UNC Lenoir Health Care, reported that its business associate (BA) was the victim of a security incident that compromised the protected health information (PHI) of 5,007 individuals. The PHI involved included names, Social Security numbers, addresses, telephone numbers, email addresses, dates of birth, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also offered complimentary credit monitoring services and created a toll-free number for questions or concerns.
