UCare Minnesota, the covered entity (CE), reported that numerous employees were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 4,806 individuals. The ePHI involved included names, addresses, birthdates, claims and financial information, medications prescribed, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its sensitive data. The CE also implemented a fraud claims review process for impacted members to monitor individuals’ claims for potential inappropriate billing. During the investigation, OCR provided technical assistance to the CE regarding its Security Rule and Breach Notification Rule policies and procedures.
