Cognizant's TriZetto Provider Solutions said attackers accessed a web portal from November 2024 until October 2025, exposing SSNs, insurance member IDs, Medicare identifiers and health data of 3,433,965 people. Providers were told in December 2025 and consumers from February 2026.
