The PHI involved included names, dates of birth, Social Security numbers, drivers’ license numbers, addresses, diagnoses/conditions, and other treatment information. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and retrained workforce members to better protect its PHI.
