The covered entity (CE), Total Diagnostix II, LLC, reported that a business associate (BA) impermissibly accessed the CE’s computer system and downloaded the electronic protected health information (ePHI) of approximately 855 patients. The ePHI involved included names, addresses, birthdates, diagnoses, and lab testing results. The CE provided affected individuals with complimentary credit monitoring services. Consequently, OCR was notified that the CE is no longer in business.
