The covered entity (CE), Texas Spine Consultants, reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 8,048 individuals. The PHI involved demographic and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach the CE implemented additional administrative and technical safeguards to better protect its PHI.
