Due a vulnerability on the covered entity’s (CE) website, a database containing protected health information (PHI) was potentially accessible to individuals who bypassed existing security safeguards. The breach involved the PHI of approximately 7,376 individuals and included demographic and clinical information. The CE provided breach notification to HHS, affected individuals, and the media. Following the breach, the CE immediately fixed the misconfiguration and tested and validated its security configuration. OCR reviewed the CE's policies for compliance with the Security Rule.
