Surgery Group SC

The covered entity (CE), Surgery Group SC, reported that its business associate (BA) was the victim of a ransomware attack affecting the protected health information (PHI) of 7,646 individuals. The PHI involved included names, addresses, dates of birth, diagnoses, conditions, medications, lab results, claims information, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the BA implemented additional technical safeguards and revised its policies and procedures.
