Sunrise Treatment Center

The covered entity (CE), Sunrise Treatment Center, LLC, reported that an employee was the victim of an email phishing scheme that compromised the protected health information (PHI) of 3,660 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses, and medications prescribed. The CE notified HHS, affected individuals, the media, and provided substitute notice. Complimentary credit monitoring services were provided. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. Workforce members were retrained on email security.
