Stokes Regional Eye Centers

Stokes Regional Eye Centers, the covered entity (CE), reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 266,170 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims information, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute breach notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring and identity restoration services to those affected.
