The covered entity (CE), Steward Medical Group, reported that an employee was the victim of an email phishing attack that affected the protected health information (PHI) of 640 individuals. The PHI involved included names, dates of birth, Social Security numbers, and claims and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained its staff on email security.
