Stern Cardiovascular Foundation

The covered entity (CE), The Stern Cardiovascular Foundation, reported that it experienced a ransomware incident that compromised the protected health information (PHI) of 270,645 individuals. The PHI involved included names, addresses, dates of birth, driver’s license or other identification numbers, Social Security numbers; diagnoses, medications, financial and claims information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. OCR provided technical assistance regarding the timeliness requirements of the HIPAA Breach Notification Rule.
