Stark Summit Ambulance, the covered entity (CE), reported that several employees were the victims of an email phishing scheme that affected the electronic protected health information (ePHI) of 3,700 individuals. The ePHI involved included names, addresses, birthdates, drivers’ license numbers, Social Security numbers, financial information, diagnoses, medications prescribed, and other treatment information. Shortly after the CE reported this breach incident, OCR confirmed that it had ceased operations. As a result, OCR closed its investigation.
