Square Medical Group

Square Medical Group, the covered entity (CE), reported that an employee accidentally sent an email communication without using the blind carbon copy function. This affected the protected health information (PHI) of 2,363 individuals. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The PHI involved included names and email addresses. In its mitigation efforts, the BA strengthened its administrative safeguards and retrained its staff on the requirement to protect and secure PHI.
