Speight Family Medical, the covered entity (CE), reported that documents scanned into its electronic medical records system were encrypted by a virus. The CE’s protected health information was not impermissibly accessed, viewed, or disclosed. Therefore, this security incident was not a breach. To address this incident and prevent future security incidents, the CE retrained its employees and strengthened its technical and security safeguards.
