Spectrum Health Lakeland

The covered entity (CE), Spectrum Health Lakeland, reported that an employee of its vendor was the subject of an email phishing scheme that affected the protected health information (PHI) of 1,100 individuals. The PHI involved included names, dates of birth, Social Security numbers, diagnoses, and claims information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect PHI. In addition, the CE retrained its workforce members on email security precautions.
