Sparta Community Hospital District

The covered entity (CE), Sparta Community Hospital District, reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 909 individuals. The PHI involved included names, birthdates, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect PHI. Staff were retrained on email security.
