Southern Ohio Medical Center, the covered entity (CE), reported that its business associate (BA) experienced a cyber-attack affecting 1,333 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, telephone numbers, e-mail addresses, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary identity and credit monitoring services and implemented additional administrative and technical safeguards. Staff were retrained.
