Southeast Nursing and Rehab Center, the covered entity (CE), reported that medical records maintained at an offsite storage facility were inadvertently auctioned off and made available to unauthorized individuals, which compromised the protected health information (PHI) of 2,721 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license and Social Security numbers, claims and financial information, diagnoses, lab results medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its administrative, technical, and security safeguards.
