South Country Health Alliance

The covered entity (CE), South Country Health Alliance, reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 66,874 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, diagnoses, medications, and other treatment information. The CE notified HHS, the affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services, established a call center for questions or concerns, and implemented additional administrative, technical and security safeguards. Staff were retrained.
