The covered entity (CE), Skin Care Specialty Physicians, reported that a work bag containing the protected health information (PHI) of 1,553 individuals was stolen from an employee’s car. The PHI involved included names, dates of birth, phone numbers, and other clinical information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative safeguards and sanctioned the employee responsible for the breach. The CE also provided training on the need to safeguard PHI.
