The covered entity (CE), siParadigm, reported that the protected health information (PHI) of 1,654 individuals was viewable via the Internet. The PHI involved included names, dates of birth, case numbers, and clinical information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative safeguards and retrained its staff.
