The covered entity (CE), Sioux Falls Veterans Administration Health Care System, reported that an employee emailed the protected health information (PHI) of 564 individuals to the wrong recipients. The PHI involved included names, addresses, and treatment information. The CE notified HHS, affected individuals, and the media. The CE also implemented administrative safeguards and retrained its staff. OCR obtained assurances that the aforementioned corrective actions were implemented.
