The covered entity (CE), Sinai Health System, reported that several employees were victims of an email phishing attack that compromised the protected health information (PHI) of 12,578 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, drivers’ license numbers, diagnoses/conditions, medications prescribed, and financial and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards and provided training to its staff on cybersecurity issues.
