Shelby Dermatology d.b.a Dermatologists of Birmingham

The covered entity (CE), Shelby Dermatology d.b.a. Dermatologists of Birmingham, reported suspicious activity within its network environment that affected the protected health information (PHI) of 86,414 individuals. The PHI involved included demographic and clinical information. In response to the breach, the CE provided complimentary credit monitoring services to the affected individuals and implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
