The covered entity (CE), Servicios de Salud Primarios de Barceloneta, reported that employees were the victims of an email phishing scheme that compromised the protected health information (PHI) of 60,200 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license numbers, Social Security numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
