Serenity Care PACE

The covered entity (CE), Serenity Care PACE, reported that its business associate (BA) was the subject of a ransomware attack that affected the protected health information (PHI) of 658 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, and photographs. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. Staff were retrained.
