The business associate (BA), Sequoia Concepts, reported that it experienced a ransomware attack affecting the protected health information (PHI) of 2,230 individuals. The PHI involved included names, Social Security numbers, addresses, dates of birth, and claims information. The BA notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services to affected individuals and implemented additional administrative and technical safeguards.
