Sentara Healthcare, the covered entity (CE), reported that its business associate (BA) experienced a ransomware attack affecting the protected health information (PHI) of 72,121 individuals. The PHI involved included addresses, Social Security numbers, dates of birth and other identifiers. The CE notified HHS, affected individuals and the media. OCR provided the CE with technical assistance regarding the Breach Notification Rule.
