The covered entity (CE), Senior Choice dba The Atrium; Beacon Ridge; and The Patriot, reported that its business associate (BA) was the target of a ransomware attack that affected the protected health information (PHI) of 5,057 individuals. The PHI involved included names, Social Security numbers, addresses, drivers’ license numbers, dates of birth, diagnoses, lab results, medications, and financial and claims information. The CE notified HHS and the affected individuals. In response to the breach, the CE and BA implemented additional administrative, technical, and security safeguards to better protect PHI.
