Seneca Nation Health System, the covered entity (CE), reported that it was the victim of a cybersecurity incident that affected the protected health information (PHI) of 12,000 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and Social Security numbers, claims and financial information, diagnoses/conditions, lab results, medication information, and other treatment information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data and retrained staff.
