Santa Rosa & Rohnert Park Oral Surgery

The covered entity (CE), Santa Rosa & Rohnert Park Oral Surgery, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information of 5,357 individuals. The ePHI involved included names, addresses, birthdates, Social Security numbers, drivers’ license numbers, claims and financial information, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. In response to the breach, the CE retrained its staff on the proper methods of identifying and responding to fraudulent email communications and implemented additional technical safeguards to better protect its ePHI.
