RXN, Inc., dba Lancaster Pharmacy, the covered entity (CE), reported that documents containing the protected health information (PHI) of 856 individuals was stolen during looting due to civil unrest. The PHI involved included names, addresses, birthdates, and prescription information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Complimentary credit monitoring services were provided to all affected individuals. In its mitigation efforts, the CE implemented additional physical and security safeguards to better protect its sensitive data.
