MiraMed Global Services, Inc., a business associate (BA), reported than an employee improperly disclosed protected health information (PHI) with the intent to sell it to an unauthorized third party. The employee compiled protected health information (PHI) and sent it to his personal email account. The PHI involved included names, addresses, dates of birth, Social Security numbers, and insurance information. This breach affected 49,201 individuals. The covered entity (CE), Rush University Medical Center, notified HHS, affected individuals, and the media. Following this breach, MiraMed sanctioned the employee and notified local law enforcement. Additionally, The BA implemented additional administrative and technical safeguards to better protect PHI and retrained its staff. OCR obtained assurances that the BA implemented the corrective actions noted.
