Rosenfeld VanWirt

The covered entity (CE), Rosenfeld VanWirt, reported that its business associate (BA) was the victim of a ransomware attack that affected the protected health information (PHI) of 18,719 individuals. The PHI involved included names, dates of births, addresses, Social Security numbers, drivers’ license numbers, diagnoses, lab results, medications, and other treatment information. The BA notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards and provided complimentary credit monitoring services to affected individuals.
