Rodeo Pharmacy

The covered entity (CE), Rodeo Pharmacy, reported that an employee emailed documents that contained the protected health information (PHI) of 1,574 individuals to his personal email account. The PHI involved included names, addresses, telephone numbers, dates of birth, medication information, and health insurance information. The CE notified HHS, affected individuals, the media, and local law enforcement. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI.
