On February 17, 2017, Robert E. Torti, MD, PA d/b/a Retina Specialists, the covered entity (CE), reported that the protected health information (PHI) of 887 individuals went missing. The types of PHI involved in the breach included clinical, demographic, and financial information. The CE provided breach notification to HHS, affected individuals, and the media. It also notified law enforcement. Additionally, OCR obtained and reviewed evidence that the CE implemented improved administrative and physical safeguards, enhanced physical security measures, revised procedures for handling PHI, and retrained staff.
