The covered entity (CE), Riverview Health, reported that its business associate (BA), Priority Press, Inc., inadvertently emailed the protected health information (PHI) of 2,610 individuals to the wrong recipients. The PHI involved included names and addresses. The CE notified HHS, affected individuals, and the media. The CE also provided substitute notice. In its mitigation efforts, the BA implemented additional administrative safeguards and retrained its staff. The responsible employee was sanctioned. Of note, the CE also provided HIPAA training to its entire workforce. OCR obtained assurances that the BA and CE implemented the corrective actions noted.
