Rim Country Health and Rehabilitation

The covered entity (CE), Rim Country Health and Rehabilitation, reported that a workforce member was the subject of an email phishing scheme that compromised the protected health information (PHI) of 720 individuals. The PHI involved included names, addresses, social security numbers, dates of birth, claims information, diagnoses/conditions, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided additional employee training.
