Resource Anesthesiology Associates of IL

The covered entity (CE), Resource Anesthesiology Associates IL, reported that its business associate (BA) was the victim of a ransomware attack that affected the protected health information (PHI) of 18,262 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, diagnoses, and medications. The CE notified HHS, affected individuals, the media, and provided substitute notice. During the course of OCR’s investigation, the CE confirmed that it is no longer in business; therefore, the investigation was terminated.
