The covered entity (CE), Rehabilitation Support Services, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 23,907 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, financial and health insurance information, medication information, diagnoses, and lab results. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its PHI. Employees were also retrained.
