The covered entity (CE), Rehabilitation Hospital of Northwest Ohio, reported that numerous employees were the victims of and email phishing scheme that affected the electronic protected health information (ePHI) of 1,143 individuals. The ePHI involved included names, dates of birth, health insurance information, Social Security numbers, drivers’ license numbers, and treatment information. The CE notified HHS, affected individuals, and the media. The CE investigated the incident, implemented new policies and procedures, and initiated new security measures.
