Region IV Area Agency on Aging

The covered entity (CE), Region IV Area Agency on Aging, reported that an employee was the target of an email phishing scheme that affected the protected health information (PHI) of approximately 3,171 individuals. The PHI involved included names, addresses, Social Security and drivers’ license numbers, dates of birth, financial information, and diagnoses/conditions. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE instituted mandatory phishing awareness training with monthly attack simulations, reset passwords and deployed additional administrative and technical safeguards to better protect its PHI.
