Radiation Oncology Centers of the Carolinas

The covered entity (CE), Radiation Oncology Centers of the Carolinas, reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 1,116 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, diagnoses, lab results, medications, claims and financial information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on email security precautions.
