Puerto Rico Women and Children’s Hospital, the covered entity (CE), reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 99,943 individuals. The ePHI involved included names, addresses, birthdates, Social Security numbers, drivers’ license information, claims information, financial data, diagnoses, lab results, medications prescribed, health insurance information, clinical information, and treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary identity theft services. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards. The responsible employee was sanctioned. Unfortunately, the CE paid a ransom to restore missing patient data. OCR obtained assurances that the CE implemented the corrective actions noted.
