Puerto Rican Organization to Motivate, Enltighten, and Serve Addicts

The covered entity (CE), The Puerto Rican Organization to Motivate, Enlighten, and Serve Addicts, Inc., reported that an employee was the victim of an email phishing scheme that affected the protected health information (PHI) of 33,933 individuals. The PHI involved included names, dates of birth, Social Security numbers and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE sanctioned the workforce member and implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications.
