The covered entity (CE) PruittHealth Home First, Inc., reported that an unencrypted laptop containing the protected health information (PHI) of 4500 individuals was stolen from an employee. The PHI affected by the breach included individuals’ names and clinical information, such as diagnoses and conditions.
