The covered entity (CE), ProPath Services, reported that several employees were victims of an email phishing attack that affected the electronic protected health information (ePHI) of 39,213 individuals. The ePHI involved included names, dates of birth, lab results, diagnoses, clinical information, Social Security numbers, financial information, drivers’ license numbers, health insurance information, and passport numbers. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE retrained its employees on the proper methods of identifying and responding to fraudulent email communications. The CE also strengthened its administrative, technical, and security safeguards.
